Serve Minnesota Program

Serve MN: Real Help from Lawyers for Families Dealing with Dementia

What is Serve MN?

Serve MN is a program created and operated by the Elder Law Section in partnership with the Alzheimer’s Association of Minnesota and North Dakota to provide important information and resources to individuals recently diagnosed with Alzheimer’s and to their families. Through Serve MN, individuals and their families will receive basic information about long-term care planning, estate planning and end-of-life planning from a volunteer attorney at no cost to the individual.

How Does Serve MN Work?

Individuals suffering from memory loss and their family members or care partners often attend Alzheimer’s Association care consultations to receive emotional support and basic information on understanding Alzheimer’s, treatment options, general information about aging and brain health, and finding ongoing care from qualified professionals.

Care consultants will also provide the individual with information about the Serve MN program. Upon request, the individual will be given the name of a volunteer attorney. The volunteer attorney will meet with the individual, either by phone or in person, and provide the individual with information about his or her need for legal services in connection with an Alzheimer’s diagnosis. The volunteer attorney will also provide the individual with a list of legal resources to help him or her pursue additional legal help.

How Can I Get Involved?

The Elder Law Section of the Minnesota State Bar Association thanks you for your interest in Serve MN.  If you would like to learn more about Serve MN and the requirements for participation, please download and read the “Information for Volunteer Attorneys” form below. Volunteer attorneys may participate in the program upon completing the Enrollment Form below, certifying that the attorney meets the program requirements, and emailing it to ICAServeMN@gmail.com. Once the completed form is returned to us, you will be added to the volunteer attorney referral list.

Thanks for volunteering with Serve MN to help individuals diagnosed with Alzheimer’s disease pursue peace of mind and legal protection of their rights and well-being.  Your service is very much appreciated.
