The MSBA offers legal forms to its members through its practicelaw.org website. Current MSBA members may follow the prompts to set or reset their password, using the e-mail address on file with the MSBA.

For the public
Real Estate Forms
The Residential Real Estate Committee of the MSBA Real Property Law Section has prepared several sample documents for the public. Current titles:

• Minnesota Standard Residential Lease
• Contract for Repair, Replacement, Restoration, or Remodeling

Health Care Directive Form
The MSBA also offers a Health Care Directive form that allows adults to dictate in advance what medical procedures may be used on them. We urge all adults to fill out the form, have it witnessed and make sure your family understands your wishes.
